A brand new NASA report says that the deaths of the astronauts on-board the Columbia space shuttle were caused by a variety of reasons. The seat restraints, suits and helmets were not properly working. Besides for this, five events happened that were fatal for the crew considering the hole in the craft's left wing which happened during the launch.
